We have more than 20 years of experience working with and supporting aging adults and adults with disabilities, along with their families, caregivers and support teams. Our role is to aid individuals to direct and manage their supports and services to continue living in their homes and thriving in their neighborhoods and communities.

The ability for a person to plan how they want to live their lives with the support of chosen family, friends, or agency assistance.
AUTHORITY
The ability for a person to have control over authorized services, funding and identify who provides his or her supports.
SUPPORT
The collaboration of resources (including natural supports) to assist a person in living a meaningful life in the community.
RESPONSIBILITY
The recognition of a person’s contribution through employment, relationships, spiritual development, and general caring for others, as well as accountability for spending public dollars in ways that are life-enhancing.

We maintain an extensive database of Direct Care Professionals that Participants can access whenever they need services, creating new opportunities for you. We also make it simpler for you to file Electronic Visit Verification (EVV), receive payments, and keep your skills up to date with training programs. If you’d like to take on additional responsibilities in your community, please add your name to our database.
